Critical Analysis of the Amniotic Fluid Shake Test

Abstract

Multiple studies were performed on 121 amniotic fluid samples obtained from 119 women. The fluid was analyzed for its Iecithin-sphingomyclin (L/S) ratio, foam stability, bilirubin ({Delta}ODco,), creatinine, and amniotic fluid/blood creatinine ratio. In addition, comparisons were made with the neonatal outcome. It was found that in comparison to the L/S ratio, the foam or shake test had a false-positive rate of 18% and a false-negative rale of 7%. This was found to be as sensitive and specific or better than the other fluid parameters. Of 7 infants who developed respiratory distress syndrome (RDS), 5 were predicted by both the foam test and L/S ratio and, although 2 had mature amniotic fluids, complicated deliveries probablv contributed to a transient mild RDS.
