In New York City from July 1, 1970, through June 30, 1972, there were 49,474 intraamniotic instillations of hypenonie saline for midtri-mester abortion. Ten of the patients died. In this presentation the causes of death are documented and some consideration is given to the prevention of such complications in the performance of these procedures. The causes of death were not the expected hypernairemia except in one instance. The main factors which led to death were the complications secondary to incomplete abortion with bleeding and sepsis, complications which arose from the use of other agents to shorten the instillation-abortion time, and complications secondary to preexisting medical conditions which were not compatible with this method of abortion.
