This is a report of pregnancy complicated by uterine leiomyomas and extensive metastases of histologically benign leiomyomas to retroperitoneal lymph nodes and lungs. This is believed to be the first report of metastasizing leiomyomas associated with pregnancy. After hysterectomy the pulmonary lesions regressed only to recur 1 year later. The benign microscopic appearance of this tumor belies its malignant clinical behavior. Etiologic considerations and prognostic implications are discussed.
