SUPRAPUBIC CYSTOSTOMY WITH ENDOSCOPY

A Window in the Bladder

Jack R. Robertson, MD, FACOG

From the Department of Obstetrics and Gynecology at the University of Southern California Medical Center, Los Angeles, California

The postoperative care of the urinary bladder is a major gynecologic problem. A new instrument, the Cystotrocath®, allows placing a large catheter suprapubically without making a blind puncture from above. The short urethra of the female easily accepts a catheter for bladder drainage. However, women often complain more about the discomfort and pain secondary to the catheter than they do about their surgery. The urethral catheter with its sequelae of infection, trauma and bladder spasms has been the major complicating factor of urethrovesical repair. A new method reduces these postoperative complications. Endoscopy may be accomplished. The large bore catheter, the Cystodrain®, allows an endoscope to he inserted suprapubically for intravesical viewing during the placement of sutures at surgery.
