A case of holoacardius is presented with a review of literature. The physiology and anatomy of these monsters are well known, but the etiology remains obscure. Theories of etiology are presented and discussed along with the new finding of lymphocytic and plasma cell infiltrates which suggest either immunologic competence of the holoacardius or a rejection reaction by the mother or normal twin. The clinical findings and complications are also discussed.
