From the Department of Obstetrics and Gynecology, United States Army, Tripler General Hospital, APO San Francisco 96438.
The literature pertaining to the management of the term breech is reviewed and the causes of increased perinatal morbidity and mortality are discussed. A prospective study is described in which classification criteria and management protocol were evaluated as to their effect in reducing perinatal morbidity and mortality. Using this protocol, 111 term primigravida breeches were managed, with a cesarean section rate of 21.6% and a corrected perinatal mortality rate of 0. Based on this study and the literature review, recommendations for the management of these patients are offered.
