I. A Gynecologic History Questionnaire II. A Simple Data Accounting and Retrieval Card

This article describes two forms which have proven very useful in a 3-man obstetric-gynecology office practice. The first is a gynecologic history questionnaire designed for self-administration by the patient, to save the doctors' time. It is short, simple, read easily, and is a reliable guide to further management. The second form is a simple punch card, which, along with a simple sorting needle can be used for accounting and for retrieving clinical data; in this case, diagnoses and treatments. It has proven valuable for preparing lectures and papers, and for evaluating treatment methods.
