Abstract
Primary retropubic vesicourethropexy has its advocates, but most gynecologists prefer an initial vaginal approach with concomitant correction of other pelvic floor problems. In this series of 117 such operations, the cure rate after 1 year was 76% and after 3 years, 75.3%. There were 7.7% known surgical failures. In properly selected patients and in the absence of complications, this operation may well be the best procedure for the operative treatment of recurrent stress incontinence after failure of repair by vaginal approach. Many problems remain, however; serious complications do ensue and not all patients achieve a successful result
