POSTPARTUM THROMBOPHLEBITIS OF THE OVARIAN VEIN

The seventeenth reported case of thrombophlebitis of the ovarian vein is presented. It is the first one treated by vena caval plication and ovarian vein excision. Virchow's triad—venous stasis, hypercoagulability, and endothelial injury—is established as the etiologic basis for this disease. The three factors are discussed in relation to pregnancy and the postpartum state. It is urged that the treatment of this disease should be conservative, ie, anticoagulation therapy. Operation should be restricted to patients who experience pulmonary emboli or who cannot be given anticoagulants. Vena-caval plication is the surgical treatment of choice.
